New patients are always welcome.
Our staff will be pleased to assist you in any way possible and all efforts will be made to accommodate your schedule. Everyone at our office is at your service.
Billing & Fees
Payment is requested at the time the service is provided. Patients with insurance requiring co-payment are requested to pay the co-payment at the time of the service. If you have any questions about your bill, or have a temporary financial problem, please discuss these with our patient accounts representative.

Cancellations
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. This courtesy on your part will make it possible to give your appointment to another patient.
Medicare
Our physicians are Medicare participants. Your Medicare benefits apply when a medical diagnosis is made. Refractions for glasses are not a Medicare benefit.
Insurance
The Hillsboro Eye Clinic participates in most health care plans. We will assist in the completion and filing of your insurance claims, or if you wish, we will provide a statement containing all the necessary information for you to submit your claim. To process insurance claims, we need to obtain all current insurance information at the time of your appointment.
 CareCredit is a flexible patient/client payment program, specifically designed for healthcare expenses, that makes it easier for you to get the treatment or procedures you want and need. CareCredit is ideal for co-payments, deductibles, treatment and procedures not covered by insurance.

The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time you will save significant time during your visit. If you have any questions when filling out these forms, please do the best you can, our staff will assist you with your questions on the day of your visit.
